Enrollment Forms
If you are enrolling for after-school care at the following schools (Beutel, Roberts, Brannen, Rancho, Ney, Polk), please download, print and fill-out all nine of the forms below.
All forms need to be submitted to Boys & Girls Club of Brazoria County, 202 W. 1st Street, Freeport or faxed to 979-373-9665. The documents can be viewed and printed using Adobe Acrobat Reader.
Please do not fill out the forms below if you are enrolling at Ogg, Griffith, Frontier, West of the Brazos or the Freeport Branch. Only membership forms, parental releases and the student-parent handbook are required for these locations as they are Traditional Clubs. Please print out this membership form , parental release and parent handbook and fax the completed forms back to us at 979-373-9665.
